unp (1).jpg UPNPplus is a community-based organisation that speaks for people living with HIV/AIDS across Uttar Pradesh. It was built by and for the community itself, so the people it serves also help decide what it does. Its work brings treatment, care and dignity closer to families who are often pushed to the edges of the health system. It works through a participatory approach, which means members of the community lead the outreach rather than being visited by outsiders. This role sits within UPNPplus's CSC-2.0 SSHAKTI programme in Lucknow district, which tracks people on HIV treatment and makes sure nobody drops out of care.

Role

The Community Liaison Health worker is the person who stays in touch with people living with HIV in Lucknow district and makes sure their treatment does not stop. The work is mostly in the field: visiting homes, following up with people who have missed appointments, walking newly diagnosed people through their first months, and helping mothers and babies get the tests and care that keep the virus from passing on. It also means helping people fill the forms that unlock government entitlements they are owed.

Responsibilities

  • Track MIS and lost-to-follow-up (LFU) cases systematically and bring those people back into treatment.
  • Provide close support to newly diagnosed people living with HIV during their first three months on treatment.
  • Follow up with pregnant women to prevent vertical transmission, covering treatment adherence, ANC check-ups, institutional delivery and post-natal care.
  • Follow up on infants up to 18 months, making sure life-saving drugs are given and the EID test schedule is kept for HIV-free certification.
  • Link clients who test reactive at ICTC to the nearest ART Centre so treatment starts quickly.
  • Conduct follow-ups with sero-discordant couples.
  • Monitor people on second-line or third-line ART, watching health outcomes and reducing the risk of transmission.
  • Follow up with people undergoing ATT, supporting them through both ART and ATT treatment.
  • Work with clients to improve treatment adherence and retention, aiming for over 80% adherence.
  • Check in with stable clients, collect health status updates and assess what services they still need.
  • Help households fill social entitlement forms, collect them and hand them to the Project Coordinator.
  • Work with the CSC 2.0 team on timely programme reports, compiling and validating data in the given tools and supporting outreach staff with field data recording.

Requirements

  • Completed 12th standard education.
  • Basic understanding of the National AIDS Control Programme (NACP).
  • Willingness to work with and within groups of people living with HIV/AIDS.
  • Reasonable writing skills for records, forms and reports.
  • Reasonable speaking skills for talking with clients and their families.
  • Candidates from the community of people infected or affected by HIV/AIDS are preferred.
  • Comfortable keeping confidential health information private.
