Field Coordinator in Keonjhar | Swasti - The Health Catalyst
Swasti - The Health Catalyst
Swasti is a public health organisation and part of The Catalyst Group. It works with governments to make healthcare easier to reach for the people who need it most. In Keonjhar, Odisha, Swasti supports the District Administration and the District Health Society to run the ABDM-Enabled Care Coordination (AECC) initiative for the State Department of Health and Family Welfare. AECC links health records, a care helpline, a district dashboard and village-level health scores into one connected system. Keonjhar is the first district to try this model, and the State plans to take it to other districts.
Role
As a Field Coordinator, you will be the face of AECC in villages and health facilities. Many families in Keonjhar live in remote, forested and tribal areas, where distance, language, missing documents and cost keep them from care. You will help these families use the new system, bring communities on board, and work with frontline health workers to clear the barriers in their way. You will also be the project's eyes and ears on the ground, listening to people and writing down what works and what needs to change, so the team can improve the model quickly.
Responsibilities
- Support the rollout of AECC in your assigned facility areas, starting with Padampur Community Health Centre and expanding to more facilities over time.
- Organise ABHA health ID enrolment camps, community orientation sessions and facility launch events.
- Help citizens use AECC services such as the helpline, appointments, referrals and follow-ups, and check that referred patients actually reach care.
- Raise awareness of AECC and ABHA through village meetings, Village Health, Sanitation and Nutrition Days, Self-Help Group meetings, schools and local events.
- Test communication materials in villages and report what people understand and what confuses them.
- Reach out to remote villages, Particularly Vulnerable Tribal Groups, women, older people, people with disabilities and households that rarely use public services.
- Identify barriers to care such as distance, transport, missing documents, language, digital skills and cost, and work with ASHAs, ANMs and the care centre to solve them.
- Connect families with government health entitlements and transport, and escalate problems that cannot be solved locally to the Project Lead.
- Record citizens' concerns and complaints, refer grievances to the care centre, and follow up until they are resolved.
- Keep clear field notes on what is working and why, and collect short community stories with people's consent.
- Build strong working relationships with ASHAs, ANMs, Community Health Officers, Anganwadi Workers, Panchayat members and community leaders, supporting their work without replacing it.
- Submit weekly field reports and a monthly summary, keep records of meetings, camps and households reached, and join team reviews and trainings.
